By signing this document, I agree to all terms and conditions listed. This agreement is entered between Kasten’s Dog Training, Inc. (KDT) and [OWNER’S NAME], here forward referred to as “owner”, under the following conditions for [DOG’S NAME] here forward referred to as “dog/(s)”.

Signee represents that he/she is the owner of the dog(s) to be boarded, trained, or transported or has the authorization of the dog’s(s’) owner to sign this document and all other contracts for services provided by KDT.
Kasten’s Dog Training, Inc. (KDT), its employees, its representatives, and its agents care for and supervise your dog when your dog is on the premises of KDT while under KDT’s control for training, including but not limited to on, or off-site, participation. KDT diligently protects the health and safety of the dog. However, the dog owner releases KDT of liability for loss of or damage to the dog or owner’s other property resulting from fire, disease, illness, virus, escape, theft, death, calamities, mishaps, or anything else outside the control of KDT.
It is agreed by the owner that KDT, its agents, officers, and employees assume no liability for the health or safety of the dog(s) that is participating in any of KDT’s services including but not limited to all on or off-site participation.
The dog owner is exclusively and fully liable and legally responsible without recourse to KDT upon the owner controlling the owner’s dog when, among other things, training, and leashing.
After KDT returns the dog to the owner, the owner agrees KDT is not responsible for liability for loss or damage arising from or caused by the owner’s dog and wholly holds KDT harmless.
Vaccinations: Owner understands that it is his/her responsibility to always provide KDT with current vet records for Rabies, Distemper, Parvo, and Bordetella. It is not KDT’s responsibility to remind owners of this responsibility. KDT reserves the right to deny services for unvaccinated dogs or any dogs that do not comply with KDT vaccination policy. NOTE: Flea and tick, leptospirosis, protection from intestinal parasites, and canine influenza are strongly recommended.
Veterinary Communication: By signing this agreement, owner hereby gives KDT written consent to obtain veterinarian records at any time and/or speak with or obtain written documentation to/from the veterinarian directly about any cases or incidents related to KDT. We recommend owners contact their veterinarian to complete medical information release forms to authorize access for KDT.
Should your dog go into heat during a training program or during a boarding stay, the owner will be notified. The boarding fee is $95 per day for each day the dog is in heat. Any training will be discontinued, and the owner will have 24 hours from notification to pick up your dog from the training program, or the service will be turned into a boarding stay. KDT cannot guarantee the dog will not become pregnant. Once your dog’s bleeding has stopped for a full 24 hours you may contact KDT to reschedule the training program. Additional training days may be required at the cost of the dog’s owner.
KDT reserves the right to deny services or to terminate services at any time as determined by KDT.
Any deposit provided is non-refundable. For training, the $200.00 scheduling deposit reserves your dog’s place in KDT’s schedule for training. Cancellations within 7 days of receipt of deposit will result in only a $100.00 refund. Owner acknowledges the deposit is non-refundable and non-transferable after 7 days. The deposit will apply toward your training fee. During KDT defined holidays or during peak period, a boarding deposit will be required.
Should you be unable to pick up your dog in the allotted program days for any reason, you will be charged an additional $85 per day. Should KDT determine your dog needs additional training days beyond the program’s allotment due to behavior modifications, aggressive personality, or shy/timid personality, you will be charged $85 per additional day. Additional days of boarding a dog who is not in training would be normal daily rates.
All training and problem-solving requires the full participation of the owner. With group lessons and private lessons, it is the owner’s responsibility to follow our instructions to train your dog to achieve your desired level of obedience, typically requiring three 15 to 30-minute workouts per day.
If training or boarding takes place during a KDT defined holiday, an additional fee of $5.00 per holiday/peak period day will be invoiced in addition to the training or boarding fee.
Owner understands and acknowledges that KDT has surveillance cameras on the premises and records surveillance.
Owner agrees to allow KDT to take pictures and/or video for promotional material for KDT purposes.
Permission to transport dog(s) upon owner request as an additional service and cost: I authorize KDT to transport my dog(s). I agree and acknowledge that in using the transportation services of KDT there are certain dangers to dogs being transported in a motor vehicle. I will not hold KDT, its owners, agents, or employees responsible for injuries sustained by my dog(s) during transportation. I further release KDT from any responsibility for my dog(s) and/or property before and after drop off and pick up. whether I am home or not when the pet is transported.
Methods of payment: Cash, personal check, Visa, and Mastercard. Training fees are due in full at your first Go Home lesson. Boarding fees are due at pick-up.
If owner returns to KDT for any additional services, KDT shall rely on the information contained in the owner information, terms and conditions, boarding contract, and/or training contract unless corrected or revised by owner. All information contained in this agreement is true and accurate and owner understands that KDT will be relying on said information in providing the agreed upon services now and in the future.
Owner understand that pricing may change with or without notice.
KDT will administer medication according to owner’s instructions. Service fees may apply for any injections of more than 2 oral or topical administrations per day.
